Statement #1 Confirmation Of Receipt Including At-Will Language I have received my copy of the Monarchs Employee Handbook. I understand and agree that it is my responsibility to read and familiarize myself with the policies and procedures contained in the handbook. I understand that except for employment at-will status, Monarchs can change any and all policies or practices at any time. Monarchs reserves the right to change my hours, wages, and working conditions at any time. I understand and agree that nothing in the employee handbook creates or is intended to create a promise or representation of continued employment and that employment at Monarchs is employment at-will; employment may be terminated at the will of either Monarchs or myself. My signature certifies that I understand that the foregoing agreement on at-will status is the sole and entire agreement between the company and myself concerning the duration of my employment and the circumstances under which my employment may be terminated. It supersedes all prior agreements, understandings, and representations concerning my employment with Monarchs.
Statement #2 Confirmation of Receipt Excluding At-Will Language I confirm that I have received a copy of the Monarchs Employee Handbook and understand that it sets forth the terms and conditions of my employment as well as the rights, duties, responsibilities and obligations of employment with Monarchs. I understand and agree that it is my responsibility to read and familiarize myself with the provisions of the handbook.
Statement #3 Confidentiality Agreement The employee acknowledges that the company shall or may in reliance of this agreement provide employee access to trade secrets, lesson plans, training procedures, customer information and other confidential data and good will. Employee agrees to retain said information as confidential and not to use said information on his or her own behalf or disclose same to any third party. This agreement shall be binding upon and inure to the benefit of the parties, their successors, assigns, and personal representatives.
Statement #4 & #5 Statement of Acknowledgement and Agreement: Monarchs Participant Safety Policies USA Gymnastics (USAG) Participant Welfare Policy & US Olympic Committee: SAFESPORT Participant Safety Handbook for Coaches, Staff, and Volunteers
I have read and been informed about the content, requirements, and expectations of Monarchs with respect to participant welfare and safety. I have received a copy of the Welfare Participant Policy and Participant Safety Handbook and have been directed to review all of Monarchs’ participant safety policies, and agree to abide by the policy guidelines as a condition of my employment and my continuing employment at Monarchs.
Further, I understand and acknowledge that Monarchs’ Participant Safety Handbook and Participant Welfare Policy and other participant safety policies may be modified at any time, and that any guidelines may be amended, revised, or eliminated at any time by Monarchs. I understand that it is my responsibility to review new guidelines that are created and distributed as well as manual guidelines that are changed or deleted.
I understand that if I have questions, at any time, regarding Monarchs’ participant safety policies, I will consult with my immediate supervisor, club administrator, or member of Monarchs’ Participant Safety Committee.
Please read the Participant Safety Handbook and Participant Welfare Policy carefully and review all of Monarchs’ participant safety policies, located in each office, to ensure that you understand Monarch’s Participant Safety Policies before signing this document.
I hereby acknowledge receipt of the Monarchs Participant Safety Policies: USAG Participant Welfare Policy and US Olympic Committee Safesport Participant Safety Handbook to prevent misconduct in sport, including child physical and sexual abuse.
